- Application (see example form), signed by the person whose certificate is requested, containing his/her full name, date and place of birth, place of residence in Italy, and the indication of the type of certificate requested, i.e. “General”, “Civil”, “Criminal” or “Pending Charges” (a General Certificate includes all civil and/or criminal entries). The Surname and Christian names of persons born abroad, should be written in block letters and the surname underlined;
- the birth certificate or the photocopy of a valid identity document of the person concerned;
- the sum of Euro 21.09 (of which Euro 14.89 for stamp duties and Euro 3.10 for clerical fees, Euro 3.10 priority service) for each PENAL (general or pending charges) certificate. The money must be sent in the form of international bank cheque (preferred) or postal order payable to “Casellario Giudiziale-Procura della Repubblica” of the headquarters of the Province in which the applicant resided in Italy (addresses can be found at http://www.giustizia.it/uffici/info/procure.htm);
- a self addressed envelope together with 2 international postage coupons.
